You can complete this form online. You can also download the form, print and complete it, then return it in person or by mail to the school office. To obtain a copy of the form, use the link at the right.

If you are still in contact with other St. Bernard’s School graduates or former parents, please let them know about this and encourage them to register with us too!

• If you are not a graduate but one or more of your children graduated from St. Bernard's School, click on Parents.
